Enrollment
In order to enroll your student in our school you will need the following documents:
- Completed Emergency Card
- Completed Registration Form
- Completed Home Language Survey
- Completed Report of Health Examination for School Entry. (Kinder/TK only)
- Completed Oral Health Assessment Form (Kinder/TK only)
- Proof of address (utility bill, rental agreement, mortgage statement)
- Copy of complete Immunization Record
- Copy of Birth Certificate
